ARTHRITIS SOCIETY

Description: The program provides treatment for individuals with arthritis and fibromyalgia using the self-management philosophy. It provides both an initial and final assessments, graded exercise program specific to each client’s physical activity, offers audio-visual and video information, literature on pain and stress management, provides other community resources and referrals to other health care professionals as needed.



ASTHMA SOCIETY OF CANADA

Description: The Society provides a variety of programs related to asthma. The public education program provides vital information about asthma including proper treatment as well as common asthma triggers. A support group offers a means of sharing information to become more knowledgeable about the disease. Call the Asthma Hotline for a group in your local area. Research funding is provided for clinical research, health care delivery and education.



BELL CANADA (Bell Canada Relay Service)

Description: The Relay Service operator is available to assist in placing or receiving calls to/from persons who use a TTY/Teletypewriter. Other services include VCO (voice carry over), HCO (hearing carry over) and reference of calls for the deaf.



BRAIN TUMOR FOUNDATION OF CANADA

Description: The Foundation provides funds for brain tumor research, patient and family support services and public education. Support groups are provided throughout Ontario for those (adults) affected by a brain tumor, their family, friends and caregivers. Information packages are available.

Description: Dedicated to providing assistance and support to those suffering from urinary incontinence or loss of bladder control. Distributes a newsletter and other literature on topics such as bed wetting, has lists of local specialists.

Description: The program raises funds for research, provides education to patients and their families and promotes public awareness.



CANADIAN HEARING SOCIETY (
Hearing Health Care Program)

Description: The Canadian Hearing Society provides a wide range of services to meet your needs. Services are offered in a fully accessible and supportive environment by professionals experienced in meeting the needs of deaf, deafened and hard of hearing people. Services include home visits, information and referral, technical devices, hearing aids, adjusting to hearing loss with Hearing Help Classes, community presentations and in-service training. Home support services for seniors living in their own home/apartment who are experiencing difficulty in independent living and communication as a result of hearing loss.



CANADIAN NATIONAL INSTITUTE FOR THE BLIND

Description: CNIB is a primary resource for information and services relating to vision health and vision loss. We provide tips on general eye health, offer information on some of the common causes of vision loss and provide facts on common eye problems.

CROHN’S AND COLITIS FOUNDATION OF CANADA (THE)

Description: The local chapter of a national non-profit, volunteer organization is dedicated to finding the cause and cure for inflammatory bowel disease and ulcerative colitis. Volunteers help raise funds for research by participating in fundraising programs scheduled throughout the year. Educational materials are available to clients, their family members and health care professionals.



HELP FOR HEADACHES OF WINDSOR

Description: HFH is a Canadian charity offering education and support for sufferers of migraines, tension-type, cluster and rebound headaches, head injury, sinus and menstrual headaches in the Windsor-Essex County area. The services include a quarterly newsletter, support groups, community presentations, one-to-one counselling, information booths, bi-yearly migraine conferences and a lending library resource centre. A physicians’ newspaper features scientific articles of headaches and their treatments.

Description: The society assists in the fight against Huntington’s Chorea (a hereditary neurological disease) and provides information and support to clients and family members.



NATIONAL ME/FM ACTION NETWORK

Description: Patients may be diagnosed with chronic fatigue or myalgic encephalo-myelitis if they experience the following symptoms: Extreme fatigue for six months or more which reduces one’s activity levels by 50%, memory and concentration loss, sore muscles, joint pain, headaches, un-refreshing sleep, fatigue for more than 24 hrs after exercise, sore throat or tender lymph node glands. Counselling services and information and referral services are available.

PARKINSON SOCIETY OF CANADA

Description: Parkinson Society Canada is a not for profit, national charitable organization. The Society raises money through corporate sponsorships, public donations, and planned gifts. Finding the cause and cure for Parkinson's disease remains our chief mission. However, while we wait for answers, we must not forget the patients and caregivers. New publications, increased patient conferences, clinical assistance programs, as well as other projects also require your support and funding. While we have come a long way towards raising Parkinson's disease in the consciousness of Canadians, we have more to do.



WAR AMPS CANADA (Southwestern Ontario Branch)

Description: An advisory service for persons who may have suffered the loss of a limb or limbs. Information and demonstration for individuals and families concerning the best type of orthopedic appliance based on data gathered by the War Amputations of Canada and its prosthetics committee in Ottawa. Service also offers Child Amputee (CHAMP) Program which provides financial assistance, transportation costs and referral for services rendered to a child amputee.

Description: The Centre offers information and support to women with breast cancer. Offers a comprehensive library and resource centre open to the public, training, workshops and networking opportunities for support group facilitators. Information about breast cancer support groups in other locations can also be obtained. These services are provided by breast cancer survivors.



WINDSOR-ESSEX COMMUNITY CARE ACCESS CENTRE (CCAC)

Description: Health care coordination service -- coordinates a wide range of in-home services to eligible clients including case management, nursing, physiotherapy, occupational therapy, social work, personal support, nutritional counselling, speech language pathology and medical supplies and equipment. CCAC provides placement in long-term care facilities, home and school health, services for children with special needs, oncology program for terminally ill, assessments for government funded day away programs and supportive housing services. Case Managers determine eligibility for drug coverage under the Ontario Drug Benefit (ODB) Plan.



WINDSOR-ESSEX THERAPEUTIC RIDING ASSOCIATION

Description: Under careful supervision, horseback riding provides physiotherapy, psychological and recreational therapy and motivation for the disabled. It strengthens and relaxes muscles, improves posture, balance, coordination and increases joint mobility. The program uses accredited therapeutic riding instructors and therapists.
